We all know that the first house where the fetus stay was the mother's womb. There are two neighbors around the house, the front is the bladder and the back is the rectum. The uterus, bladder, and rectum are within pelvic cavity, and there is a foundation, which is the pelvic floor muscle.
What is the pelvic floor muscle?
The pelvic floor muscle refers to a group of muscles that cover the lower part of the pelvis, including the levator ani muscle, the anal sphincter, and the obturator muscle.
The pelvic floor muscles are divided into superficial, middle and deep layers, each layer consisting of different muscles. The deep levator ani muscle and the coccygeus muscle work together to maintain the stability of the pelvic structure. The levator ani muscle is the most important supporting structure of the pelvic floor. It is a pair of triangular muscles with bilateral symmetry. Located at the back of the pubic symphysis. It ends at the tailbone, the anal ligament and the perineal center.
The pelvic floor muscle is at the bottom of the pelvis. Although the word "foundation" is used to describe it, the pelvic floor muscles are not unshakable. Instead, they are like a hammock under our pelvis, maintaining a peaceful home of three neighbors (uterus, bladder, rectum).
There are several outlets within the pelvic floor muscles, the behind is for the rectum to discharge waste and carbon dioxide. The front is the outlet for the bladder to drain. Another one is the passage for the delivery of newborn.
Most of the time (about 70%), pelvic floor muscles are responsible for maintaining the stability of the foundation, while (about 30%) of the time it is relaxed , this is the time when the bladder,rectum are working. Another special occasion is when the baby is coming!
During pregnancy, the size of the mother's uterus is small initially, only the size of her fist. When the fetus grows, the uterus expands. Around 6-7months old, the uterus also expands to a greater extent, and then it will be forced into the bladder. At the same time, as the weight of the fetus increases, the weight of the pelvic floor muscles increases.
During delivery, the size of the fetus is large and will cause the pelvic floor muscle to be excessively stretched.
Why does the pelvic muscle excessively stretch matter? Just like a rubber band, if it is stretched for a certain period of time, the rubber band will become inelastic, and the elasticity of the rubber band will be damaged, causing the elasticity to be insufficient or weak.
The pelvic floor muscles are also the same. If they are not repaired in time after being damaged, they will become harden and affect the structure of the uterus, bladder and rectum. Sometimes, when the pelvic floor muscle is damaged, it will give an alarm signal - such as tenderness. If you receive these signals, be sure to repair the pelvic floor muscles in time.
Why urine leakage during laughing ?
Let's take a look at the causes of leakage of urine in postpartum women.
The most important support structure of the pelvic floor is mentioned above - the levator ani muscle. The levator ani muscle supports the proximal urethra and the base of the bladder like a hammock. When the abdominal pressure is increased, bladder neck and the proximal urethra contract the levator ani muscle of the pelvic floor provides a stable base for the anterior wall of the vagina which is conducive for the closure of urethra.
In other words, under normal circumstances, when our abdominal pressure increases, the pelvic floor muscles will automatically resist the pressure exerted by the abdominal pressure on the bladder pelvic organs
In pregnancy, chronic cough, long-term abdominal pressure can cause pelvic floor muscle weakness or damage, it will cause defect in proximal urethral support structure and vaginal anterior wall , resulting in stress urinary incontinence.
Pathological mechanism of stress urinary incontinence: Stress urinary incontinence is a urinary tract dysfunction due to abnormal urinary tract control mechanism, the intravesical pressure is greater than the intraurethral pressure when the abdominal pressure is increased, and the urine is involuntarily leaked.
Urine leakage during laughing means urine ooze or leak out unconsciously which is considered stress urinary incontinence. Stress urinary incontinence means that urine is not controlled and leaks out of the body. Stress urinary incontinence is diagnosed when the patient develops urinary leakage when the abdominal pressure is elevated , and is not accompanied by urinary urgency and urge incontinence,
